• Education:

• Certification (Mandatory): One of the following from AAPC/AHIMA: CRC, CPC, CIC, COC, or CCS
• Knowledge Required:
  • Anatomy & Physiology
  • Medical Terminology
  • ICD-10-CM Coding
  • CMS Risk Adjustment & HCC Guidelines

Job Responsibilities

• Review and evaluate medical records to verify care plans and diagnosis codes
• Assign accurate ICD-10-CM codes under CMS Risk Adjustment Guidelines
• Ensure completeness, accuracy, and specificity of diagnosis coding
• Meet daily/weekly productivity and quality targets
• Apply clinical documentation knowledge to extract valid codes
• Assist in compliance with Medicare & Medicaid Risk Adjustment criteria
• Communicate and escalate documentation gaps when necessary
